SHMedics is an Innovative, preventive lifestyle provider to help NRI Parents manage chronic diseases, and reduce behavioural risk factors. SHM lifestyle plans are based on world health organization set guidelines and clinically-proven science with the focus on guiding your parents towards reaching modest life improvement and activity goals through nutrition and behavioural changes. SHM brings together the individualized attention of professional health coaches with a researched curriculum and manageable but powerful goals. 2. How are SHM Lifestyle Programs Structured?

SHM Core program is available in 3 plans Premium, Intermediate and Basic.

Awareness is the first step in maintaining and monitoring anyone`s health. SHM Programs provide first-time and repeat participants the opportunity to get a quick and easy “snapshot” of their health. SHMedics programs are designed to help improve your understanding of your parents or loved one`s health using the biometric and clinical data provided. More specifically, biometric wellness screenings:

  • Can help you understand health from the inside out and provide insights to risks and strengths that are helpful to know about.
  • Can help to find out what are doing well.
  • Can help to make the most out of your time by focusing your efforts on the behaviors that you can change to most improve your health.
  • Can help to work with your physician to take charge of you or your loved one`s health.

3. What is NRI Parental Lifestyle Mangement?

Signing your family member with SHM, gives us an insight into their current health state. The reports of this health assessment are broken down into comprehensible terms and transferred to your SHM portal where you can access them and analyze the recommended further actions.

4. What is NRI Parental Assistance?

SHM has 2 different assistance programs as stated below.

  • SHUber
  • (Finding better ways for your parents to reach hospitals, centralize medical information, and improve their lives with a Medical Officer back up)
  • In home Health Coach assistance
  • (Your Parents can now receive trained nursing care at home. In home skilled nursing can help your parents live comfortably in their home)

ELIGIBILITY

5. Who is eligible for NRI Parental Lifestyle management programs?

SHM Plans are currently available to Nonresident Indian parents who would like to have an improved enhanced lifestyle.

At SHM we have carefully designed3 different plans to suit based on age or health risks as stated below.

  • Premium Preventive Lifestyle Plan
  • (Generally Applicable for: participants above 65 yrs. Of Age or suffering with tertiary conditions)
  • Intermediate Preventive Lifestyle Plan
  • (Generally Applicable for: participants between 55 – 65 yrs. Of Age or suffering with Chronic or Pre-Existing conditions)
  • Basic Preventive Lifestyle Plan
  • (Generally Applicable for: participants below 55 yrs. Of Age, has no previous medical condition)

6. Do you guys serve remote locations?

Currently, we are serving all over AP and Telangana. We could reach any corner with help of our strong networks of Health coaches, doctors, hospitals and wellness centers.

7. How do I enroll my parents?

Enroll your parents to SHM plans in three simple steps:

  1. Sign up (2 minutes). Visit www.SHMedics.com and “Get Started”. Here you`ll enter brief contact and we will send you a email verification. Verify email & hola you just took a step towards gifting life to your parents.
  2. Once logged in (5 minutes) please provide your parents or loved ones brief details and choose the plan that suits your parent’s needs.
  3. SHM rep (within 24 hrs.) will contact you and your parents or loved ones within a day. SHM rep will pair your parents with a dedicated health coach and medical officer (either a MBBS or practicing medical personnel).

From here on medical officer and health coach will keep working on improving your parent’s lifestyle by leveraging network of doctors, hospitals and wellness centers.

8. "My Parents are Healthy" why do i need NRI Parental Assistance?

Regular health exams and tests can help find problems before they start. They also can help find problems early, when your parents/loved ones chances for treatment and cure are better. By getting the right health services, screenings, and treatments, your parents/loved ones are taking steps that help their chances for living a longer, healthier life. Their age, health and family history, lifestyle choices (i.e. what they eat, how active they are, whether they smoke), and other important factors impact what and how often they need healthcare.

Your parents might seem healthier from outside but it is always advisable for regular preventive care. In a chaos health environment like India spending few dollars and having a medical professional by your side would befit you in:

9. "We have a family doctor" why do i need NRI Parental Assistance?

NRI Parental assistance is designed to safeguard your parents/loved from end-organ damage and chronic diseases. It`s in no way replacing your parent’s current medical setup, infect it is enhancing the health and wellness experience so your parents/loved ones can live to fullest.

10. My parents/loved ones recently had blood work done. What is the use of NRI Parental Assistance?

NRI Parental assistance is designed to improve life; it’s not a blood work. We leverage blood work or annual screening to identify risks associated with their health condition. From there we design an approach tailor made to suit your parents/loved ones needs.

It is appreciated if your parents already have blood work drawn, we would review and validate the results. We would also discount the blood work/checkup cost from the plan price (which ever plan you choose).

11. My parents are travelling abroad. How will NRI Parental assistance be useful?

If you are parents are already planned to travel then we would recommend enrolling in our Basic service, which will not only give a snapshot of current health condition but also your parents/loved ones health reports will be digitized (which is always recommended to carry past history while travelling abroad). Even you are parents/loved ones abroad our medical officer will be in a constant follow up via virtual consultation.

 

12. Why Non resident Indian Parents only?

There are many reasons. Dollars is least of them.

Education and effective communication is the key to help parents/loved ones get past any concerns they may have, especially if those concerns involve their personal health information.

It’s also important to create a culture of health, regardless of whether you offer an incentive or how much it is. We found it’s easier to educate kids to take care of parents easier than explaining it to Indian population where there is a huge barrier in preventive medicine and importance of regular follow ups.

13. Will health insurance cover these costs?

No, Indian health insurance does not cover preventive medicine. But we can assist you parents with claims and guide them in all other insurance-related cost

14. How much does NRI parental plans cost?

You can choose single (1 participant), joint (2 participants) and nuclear (more than 2 participants) plans. Please view pricing by visiting pricing page under NRI Parental services.

15. I just enrolled my Parents/loved ones into SHM. What happens next?

16. My parents are enrolled into Basic/Intermediate plan. Can I have additional doctor/specialists visits?

Yes, you could book an appointment or you can book SHUber (our health cab service) for your parents/loved ones to visit health specialists.

Also, you book any additional health related services like In home physiotherapy or home dialysis through our website.

17. Why are all plans annual?

Behavioural changes have to be done by constant motivation and thorough follow-ups. So its recommended running plans for at least a year. And also, Chronic diseases which need lifetime treatment like diabetes and hypertension need constant follow-ups. For your parents to see measurably improved result and to prevent the end-organ damage they need at least 1-year time frame.

18. What will happen In home health visits to my parents/loved ones ?

During a SHMedics Health visits, a trained examiner provided by SHMedics services will collect a number of physical parameters, including height, weight, blood pressure, waist and/or hip circumference as well as a small blood & Urine specimen of you or your loved ones at your convenience. A screening is typically completed in just 10-15 minutes. The valuable information collected during the screening is used to identify health risks, and these are reported back to you in a personal report that helps you understand them so you can take action to improve your health and wellbeing.

The same reports are explained to your parents by a doctor who visits you parents/loved ones home.

MEDICAL OFFICER & HEALTH COACH

19. How do i or my parents/loved ones communicate with medical officer?

Medical Officer is available via the SHMConnect app and will provide with medical officer call number to stay in touch throughout the program/plan. The dedicated medical officer is present to guide and support your parents throughout the program. So how you or your parents communicate depends on your parent’s needs and preferences.

The same reports are explained to your parents by a doctor who visits you parents/loved ones home.

21. What is qualification of Medical Officer?

All our medical officers either have an MBBS degree or experts in the field of Clinical Research and its branches such as pharmacovigilance, Clinical Data Management, Medical and Scientific Writing.

Medical officers are accountable to oversee the development, and implementation of standards of parental/loved one care and standards of clinical practice. They will Collaborate with hospitals administrative colleagues, doctors, and other healthcare personnel for improving your parents/loved one’s life.

22. What will Health Coach interactions with my parents/loved ones be like?

Your parents will be assigned a health coach who is experienced with SHM Plans. He will provide all the help that your parents need for a healthier life including hospital visits, pharmacy delivery, in-home lifestyle visits and health specialist assistance.

The same reports are explained to your parents by a doctor who visits you parents/loved ones home.

23. What is qualification of health coach?

Our Health coaches are experts in the overall management of Nursing Practice, nursing education and professional development. He/she has overall responsibility for safe, comfortable and comprehensive nursing care of your parents/loved ones.

DIAGNOSTICS AND FOLLOWUPS

24. What is involved in the “SHM wellness screening”(Typically done in first visit)?

The process takes about 60 minutes from the point of check-in to completion. The screening includes a blood draw for many different blood tests; this blood draw can be from your arm (venipuncture) or from a finger stick and is dependent on the panel you select. Lipid and glucose tests include a blood draw for HDL “Good” Cholesterol, LDL “Bad” Cholesterol, non-HDL, Total Cholesterol, Triglycerides and Glucose. You may also have biometric measurements taken along with ECG.

25. What is involved in "Follow up visits" (Typically done after SHM wellness screening?

The process takes about 15 minutes from the point of check-in to completion and is usually done ever quarter (once every 3 months). This screening includes a collection of physical or biometric parameters that need attention.

For ex: if your parents/loved ones have thyroid dysfunction then every 3 months our health coach is scheduled to collect Thyroid profile and reviewed by a doctor. You should see an improvement quarter over quarter resulted from medical officer follow up calls (includes follow up on nutrition, pharmacy, wellness and health needs).

Also regular visits with specialist help in preventing end organ damage depending on disease pattern.

26. How does medical officer "Follow up" with my parents/loved ones?

Follow up and constant motivation is a part of SHM lifestyle plan to improve longevity or your parents/loved one’s life. Our medical officer will be in a constant follow up with your parents/loved ones to make sure that prescribed medication diet plan and wellness activities are on the check.

TIME COMMITMENT

27. What is the time commitment for SHM Lifestyle Plan?

Participation in SHM Plans requires dedicated involvement at your parents own pace. During the visits that occur every 3 months, we measure health profile that deemed important in the first complete health visit. This data collection includes biometrics, data digitization and vitals. These visits can take 45 – 60 minutes. Time spent by medical officers on implementing lifestyle changes is difficult to measure. Fortunately, SHMConnect will keep you updated with all interactions between your parents/loved ones and SHM Family.

CANCELLATION

28. What if I want to cancel my parents/loved ones enrollment?

We really think your parents/loved ones will like our program, but we understand the realities of life can make it difficult to fully commit. If you`d like to cancel before our first visit, we can close the program at no cost. After the first visit, we would retain $79/participant and will process the cancellation. Just send us a message through SHMConnect or give a call to one of SHM reps.

29. What credit/debit cards are accepted on SHMedics?

We accept VISA, MasterCard, Maestro and American Express credit/debit cards.

30. Do you accept payment made by credit/debit cards issued in countries other than USA?

Yes! We accept VISA, MasterCard, Maestro, American Express credit/debit cards issued by banks in India and in the following countries:

Australia, Austria, Belgium, Canada, Cyprus, Denmark, Finland, France, Germany, Ireland, Italy, Luxembourg, the Netherlands, New Zealand, Norway, Portugal, Singapore, Spain, Sweden, the UK and the US.

Please note that we do not accept internationally issued credit/debit cards for Flyte, Wallet and eGV payments/top-ups.

PRIVACY

31. What personal information of my parents/loved ones will be shared?

Your parent’s/loved information is only available with the medical officer and can be transferred to doctors/hospitals upon your parent’s consent.
